Disproportionately Enlarged Subarachnoid Space Hydrocephalus (DESH) is the hallmark imaging pattern of iNPH. It is characterized by ventriculomegaly, enlarged Sylvian fissures, and tightness/crowding at the high parietal convexity vertex.
Research led by Kockum et al. (2018) established that total iNPH Radscale scores >8 carry over 90% diagnostic sensitivity for iNPH in patients presenting with clinical Hakim's triad symptoms.
While the iNPH Radscale provides structured, reproducible quantification of radiological hydrocephalus features, radiological severity alone does not replace clinical CSF dynamic testing (lumbar tap test or extended lumbar drainage) for predicting shunt responsiveness.
